Support Coordination Adelaide - Support is the foundation of everything that Friendly and Graceful Care Services provide. We know you have individual needs, and we want to spend the time getting to know you and your family and fully understand how we can support you.
If support coordination is available in your NDIS plan, you can contact us to help you find mainstream and other disability services that are right for you straight away.
What is support coordination?
Support coordination is just one of the services provided by a Friendly & Graceful Care Services specialist. They will help you use your NDIS plan best by having a say in which service providers you choose.
There are three levels of support coordination available with our support coordinators in Adelaide:
Support connection: This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
Support coordination: This support assists you in building skills to understand and use your plan. A support coordinator works with you to increase your capacity to maintain relationships, live more independently and be involved in your community.
Specialist support coordination: This is a higher level of support coordination for people with complex situations. A specialist Support Coordinator will assist you in managing challenges in your environment.
How does support coordination make a difference?
Having a personal support coordinator empowers you to take control of your NDIS plan and the supports and services you receive. We listen to your needs and take the time to conduct a personal assessment and build a unique team tailored to you.
Your support coordinator will assist you to:
- Understand your NDIS plan, including
- Your support budgets and the kinds of things that can be funded
- How your funding is managed and
- What requirements you need to meet, such as when quotes might be required, what you can or cannot claim as well as how much you can claim
- Plan and coordinate your supports o Help you to find providers, services and supports.
- Help you pursue your goals and,
- Understand the mix of services and supports that would work best for you based on your circumstances, needs and preferences.
- Decide how you would like your supports and services to be delivered.
- Arrange a schedule of services that reflects your goals, needs and how you want to use your plan and fits in your NDIS plan budget.
- Connect you with supports and service
- Find NDIS, community and other government services you can access and choose the right supports and services.
- Establish and maintain your supports, including reflecting on services provided and making changes if required.
Who is eligible for support coordination services?
Support Coordination Adelaide is funded under the Capacity Building Supports budget in your NDIS plan.
